There's an out of reach ladder at the Altar of Reflection near the Stone Spire Hideout in Ghost of Yotei, but there is a simple trick to it.
Altars of Reflection are a crucial part of Ghost of Yotei. As well as being your reward for clearing hideouts, altars are by far the most common landmark dotted around the map. Each one grants you a skill point to upgrade Atsu's overall power or skill with specific weapons, and therefore it's key that you visit as many as possible.
